I second that request. It should be trivial (ie. 5 minute job) to add that
info in automatically generated list at:
(or to copy and change script to also generate users_agreed_pd.txt or
something, if chaning format of existing text file is unwanted)

I agree with this too: the checkbox should remain as it is - those who care
about license will read and decide weather they want to check it or not.
Those who don't care would eather leave it in default unchecked state (if
they are lazy kind) or would check it as "that box next to button should
usually be checked" (the inertial kind). In ideal case one can hope that
the lazy and invertial kind will mostly average each other out, thus leaving
ratio of "really want PD" and "really don't want PD" more or less correct.
However, if the improvement to the form is needed, the form could be
modified such as it doesn't have "Agree" button and "PD" checkbox, but
"Agree with PD" and "Agree without PD" buttons instead (randomly changing
order for best results [if there is the option for that], thus averaging out
"when presented with two buttons and no understanding, would users click
left or right or inform themselves" dilemma).
This improvenet could probably wait for phase3 (Existing Contributor
Mandatory Re-licensing) - when the form would need to be changed anyway
(in order for the "I disagree" button be added)
Opinions above are GNU-copylefted.
