Looks good.

I think you need to spell out what's in it for Garmin. Something like:
"A significant number of new users at mapping parties will go on to be regular OpenStreetMap contributors. They will want to purchase their own GPS receiver and if they have learned with a Garmin device at a mapping party then they are highly likely to buy a Garmin device." (the trick is to get that into the 50 word allowance).

Can you quantify the number of mapping parties that they will be used at? According to the event calendar there will be 17 mapping parties within the next month (items with a green Map logo scheduled between September 26 and October 26). You can legimately claim 200 mapping parties a year worldwide.

You could ask for a discount on sales of Garmin product to OSM members and/or a donation to OSM for each sale to an OSM member. Remind them that we have 65,000 registered users with 7,000 of them contributing every month.

I think you can be much more specific about what product you want:
20 x Garmin 60Cx

I think the term of the agreement refers to the length of time. I would say open-ended.

I'm copying in Henk Hoff who is the OSMF board member responsible for merchandising. I don't think he monitors the talk-us list.

Perhaps you could draft up your responses to the questions on the form and post them here for review. If we can achieve a well worded proposition it maximizes the chances of a favourable response.

Ok, here goes:

In 50 words or less, describe the event/organization you represent:

OpenStreetMap is an organization aimed at mapping the world with user-collected data. Contributors collect data using GPS and share it with others to create an overall map of the world. Mapping parties are gatherings to attract local people to help gather map data using handheld GPS devices. (47 words)

If products are requested, please list the model and quantity:

Handheld GPS devices with a screen displaying current location to ease with note taking, high accuracy/low drift, and the ability to save a recorded paths for later download to computer. The Garmin 60Cx seems to be the closest with the features we need. The mapping parties tend to have 10-20 people attend -- ideally we'd like one per person.

What is required from Garmin:

GPS product donation and some information or pamphlets to give out about your sponsorship at our mapping parties.

What is the proposed term of the agreement:

(I'm not sure what this is referring to. Is term in this case the length of the agreement?)
