You can draw the riverbanks in a clockwise direction tagging with waterway=riverbank. Draw both banks as part of the same way with segments along one bank going in one direction and segments along the other bank going in the other direction.
<br><br>Break the river up into manageable chunks, with one way for each chunk. Where you switch from one bank to the other and start going back up the river there is no need to join the two banks with a segment, this takes advantage (??) of the ability to have discontinuous ways.
<br><br>Islands should be drawn as part of the same way but in a counter clockwise direction. <br><br>I also draw a normal way down the centreline of the river (tagged waterway=river) for the benefit of routing, and for the benefit of renderers that want to display a river as a single line for whatever reason.

What is the official method for creating a river? The Susquehanna river runs<br>through my city. It is big and convoluted enough that I don't think marking a<br>linear way with natural=river is going to be good enough. Should I make it an
