[OSM-dev] ways with 'spurs'

I've come across some ways that go:

   A -> B -> C -> B -> D

Shouldn't they be done as:

   A -> B -> D, with a separate way B -> C?

On a more general note, is there any ongoing work to clean up the 'raw'
database so there are no zero length segments, looping/branching segments. I
know I've seen it mentioned a few times. If we can't rely on a clean
starting point I can see loads of duplicate developments for handling these
kind of cases....
