It depends on whether you measure x/y in metres or degrees.

Try Edit->Preferences. Select the 3rd item which looks like a square
grid. Try changing the projection method:

- mercator should give a square x/y grid in metres. 
- EPSG:4326 is a square x/y in degrees.

Then restart JOSM
