While browsing http://trac.openstreetmap.org/ I noticed several changes
being committed, followed by tickets for those changes being closed in a
separate operation.

If you simply put "Fixes #[nnn]" in the svn commit message, where [nnn]
is the ticket number trac will close the ticket, putting the commit
message as the closing comment, and as a bonus, the ticket will be
linked to the changeset and vice versa. If you make a change that
doesn't completely fix a ticket, but works towards it, you can use
"References #[nnn]" and the ticket will have a comment, but will not be
closed.

The above functionality is provided by:
http://trac.edgewall.org/browser/trunk/contrib/trac-post-commit-hook
