If you *don’t* moderate every message, then there still needs to be a mechanism for enforcing the CoC post-hoc.

I moderate a few mailing lists and we have four of us that get emails to push or reject, typically it's just the first person who gets to it that makes a call...we've only had three emails that I sent messages to the moderators to get their feelings on (it was actually where I thought the topic was ready to simmer down but I wasn't sure). Just throwing it out there.
