The confusion is because the trans community is using the term “informed consent” as a shorthand for a related but different concept: that HRT is available without a psychological evaluation (i.e. informed consent is the only requirement to obtain HRT). 

As Adina points out the proposed tag doesn’t say whether the tag applies to HRT only or also other related medical procedures.
