Normally, Nominatim takes care to call osm2pgsql in the right
way so that this table is created (adding the -O gazetteer
parameter).

You should follow the installation instructions on the wiki:
http://wiki.openstreetmap.org/wiki/Nominatim/Installation

If you followed these instruction and still can't find the placex table,
then you are most likely using an old version of osm2pgsql which had 
a bug in the table creation code. Get the latest version of osm2pgsql 
from subversion and build it yourself.
