[OHM] OHM Server Back Up & Updated

Hi folks,

Been working at this over the Easter period and happy to say the
OpenHistoricalMap server is back online and running!
http://www.openhistoricalmap.org/

I've updated the codebase to be running from the latest OSM website code -
so it should look nicer on the front end, and easier to maintain.

The tiles are being generated live from our API now (you may notice that
they are on a 5 to 10 minute delay though).
