Per the import guidelines, you should document this import on the wiki with its own page. And add it to the table on the imports page.
In the page you add for this, be sure to link to the license.
With respect to what tags to map into OSM, which to drop and which to import unchanged: My preference is that standard OSM tagging be used, that is “addr:street”, “addr:housenumber”, “addr:city”, and “addr:postcode” are imported and everything else is dropped. I guess if “Floor” is something other than the ground/first floor, you might translate that into a “level” tag too.
If you expect to update this from time to time, then adding the county’s unique ID as a unique tag might be reasonable so you can easily correlate them in the future. But for myself, I consider the rest of the tags to be noise.
I am slowly working my way through an address import for my county and have found that the data needs a bit of massaging. Abbreviations and capitalizations don’t always match OSM conventions or previous mapper’s work. Is that “MacArthur” or “Macarthur” or “Mac Arthur” on the street name? Is the “Main” street name in OSM correct or should it be “East Main” based on the addresses you are seeing in your import? Are you going to correct the name on the street, add an “alt_name” to the street, change the “addr:name” to match the OSM street name, or ignore the discrepancy? You might want to have a section on your import wiki page stating how you are dealing with that type of thing.
