According to the Mapnik DB 162 ways are tagged with ref='A40' and
name='A40' at the same time. 

Doing another search shows that there are several thousand entries with
name==ref in the whole database. Maybe osm2pgsql should drop the name if
ref==name and highway={primary,secondary,trunk,motorway} ?
