Modified version of OSM maps must be released under the BY-SA licence 
(you can use the same version or a later version).

For a printed map this may satisfy the "reasonableness" requirement for 
the licence. I think there may be some recommended language but I can't 
find it on the wiki.

As long as the modified work (the map itself) is BY-SA you can use it 
for anything the licence permits, which includes selling it.

If you have a visual example of what you have in mind that might help to 
find any other issues.

Attribution doesn't seem to be covered in the FAQ but there are some
