My approach to divided ways is driven (mostly) by my own laziness.
That is, I don't create parallel ways unless it could make a difference
to the data consumer.
Consider city arterials. Many of them here (Las Vegas, US) have painted
center lines. Lot's of lanes both ways but no advantage in dividing it.
The same is true when I find the little concrete islands down the middle
that have breaks for every intersection.
Some arterials have nice wide islands (perhaps with landscaping) and
only allow some intersecting streets to cross. At other intersections
the island only allows for right turns. Now I'm willing to create the
parallel ways (marked one way with matching name and ref tags). When we
have an intersection (with a break in the island) that allows cross
traffic and left turns, I connect the crossing way to both parallel ways
rather than make a short section of single way.

My hope is that data consumers (routers, renderers, etc.) will be happy
with this.
