I think that it might be a bug. I've seen this happen on some car
parks near me, too. I don't know where to report bugs like this
though. Maybe have a look on trac?

I guess simply because it's not been told to. The default Mapnik
stylesheet doesn't want to display names on everything. The data is
there in the database though so it can be used by search engines or
custom renderers.
