Try searching this discussion group. There was a discussion about this
very subject between 04/12/08 and 07/12/08 under the title:
[OSM-newbies] How to map a place that allows to cross a
waterway=stream

Looks like there's only one instance of waterway=stepping_stones,
which I too would consider wrong. You don't tag a river with a bridge,
you tag the highway.
