Yes, only the one node is modified, with no changes to the way it belongs to.

Yes, the entire way is re-uploaded including the new node (this is
required since order of nodes is important, and they can theoretically
be repeated..)

The solution, although not entirely nice, is to split the way into
two, and use a multipolygon relation to link them into one contiguous
area. http://wiki.openstreetmap.org/wiki/Multipolygon
JOSM does understand them, and should render them as expected.
