That'd be Mapdust.  Here's an example:

Skobbler (which is OSM-based routing software for some mobile phones) 
has a bug reporting option.  When someone doesn't like what Skobbler's 
telling them, they can log a bug and it appears as you see.

The quality of the reported bugs can be a bit "variable", so I'd take 
some of the reports with a pinch of salt.  Some of the reports also 
refer to Skobbler software problems rather than OSM - I'd just close 
those in Mapdust.  I also close any local ones near me that don't have 
any description of what the problem actually is (since it's not useful 
information to see on the map).

I'd also be a little wary of the "Address:" used in the Mapdust info - 
it's not sourced from OSM, so me not be freely reusable on the map.

There sort-of are, in that you can add roundabouts and turn 
restrictions.  I seem to remember a recent 
"http://help.openstreetmap.org/" answer for adding a turn restriction in 
Potlatch 2.
