If you only want to update every month or so, then yes it is probably 
easiest just to download a new complete extract. Then import it into 
your database, the same as you did originally.

This depends on what sort of tile generation script you are using.
If its just the generate_tiles.py, I don't think there is anyway of just 
rendering changed tiles. And it will skip any tiles that already exist 
in the output directory.

You could change the output directory, then it will render all of the 
tiles again.
Though this may take a while, if it needs to render tiles for a whole 
country, at a high zoom level.
