It looks like you're hitting an error in osm2pgsql loading data into 
Nominatim, not an osmosis error.

I'd suggest reviewing the instructions at http://www.nominatim.org/ to 
make sure you didn't miss a step.

If it still doesn't work, the geocoding@ mailing list would be more
suitable.
