[openstreetmap-website] Note creations fails (#346)

I am unable to create a note. When clicking on the create note icon on the right I get a browser refresh but no dialogue box that allows me to enter any detail. I have checked this on both Firefox (22.0) and Chrome (27.0.1453.110) running on Ubuntu 11.04.

It worked in the old interface if that is any clue to what has changed.
