[openstreetmap/openstreetmap-website] [WIP] Allow reporting of abusive users to moderators or admins (#1576)

I've started writing some feature tests, which let me do some more major refactoring. I'm splitting out the issues_controller into three - one for creating reports, one for viewing issues and one for managing issue comments. That makes certain things more logical, like reports#new and issue_comments#delete and so on.

Along with that I'm also fixing more bits and pieces at random when I spot them! I'll need another few stabs at this, then I'll create a task list so that we can monitor progress more easily.
