[openstreetmap/openstreetmap-website] [WIP] Allow reporting of abusive users to moderators or admins (#1576)

I think I've done about everything that I plan for this pull request, so it's now available for a more thorough review. Note that the recent batch of changes includes a change to the migration, so will need re-running.

In particular, I haven't implemented the following:
* Prevent users from submitting multiple reports
* Automatically close issues when the problem is dealt with

The latter in particular is the most pressing thing. This would mean that if e.g. a diary entry is removed by an admin, then any corresponding issue would be automatically closed. But I don't want to expand the PR any further than it already is, so these things can wait either until this PR is approved or rejected.
