[openstreetmap/openstreetmap-website] Moderation zones: clearly mark which fields cannot be edited (Issue #7389)

Similarly to https://github.com/openstreetmap/openstreetmap-website/issues/7388, moderators have limited capability to edit moderation zones. There are several restrictions which can be gathered from [test/controllers/moderation_zones_controller_test.rb](https://github.com/openstreetmap/openstreetmap-website/blob/master/test/controllers/moderation_zones_controller_test.rb) (see tests for `update` action).

Specifically for this issue: the form to edit moderation zones should clearly show which fields can be changed and which can't. Presumably with something like disabled fields or similar (but this won't be enough for the map, for example). Note that the specifics vary depending on the user who created the modzone, the person editing it, and the current status of the modzone.
