My take looking at it was that the subquery approach is just wrong - the very first version is almost certainly the best you're going to do.

That version could probably be improved a bit with an extra index but probably not a lot.

Fundamentally trying to do a high performance query that needs to use two indexes is always going to be difficult because there's just no really good way to execute such a query.

Hopefully the linestring version will be better by improving the performance of the geospatial side of the match.
