[Routing] Turn by turn instructions

A few examples of stuff I hear:
"At the roundabout, take the first exit. Then, stay in the right lane."
"At the roundabout, take the first exit."
"At the roundabout, take the first exit. Then, turn left."
All of this is more or less ok. The lanes stuff, we had the discussion,
but I don't think we're any closer to being able to implement that.
The "Then..." instructions are very helpful when driving, though,
especially if the mapping detail in the area isn't that high - if I know
I'm going to have to turn left in 200m and I have a choice of lanes, I
know to keep in the left lane even if it doesn't tell me to.
