where would you add this to? Typically if this is one or several ways rather than a single node, these would already have a tag like waterway =stream/river.
You could add these affluents to the waterway relation and add a "source" role, but it would mean adding waterways to a river relation which don't belong to the river.

IMHO for these reasons it would be better to add a tag like it was suggested in the title: a river start tag on the node where the river starts.
