[Tagging] tagging two former church locations, with the same name but unrelated to each other

I have a curious matter that I am uncertain how to tag. I have 
previously mapped the Mill Creek Baptist Church Graveyard, ID 53498031, 
and would like to add a node for the location of the former church 
building. It was demolished circa 1910, and its bricks used in the 
construction of a daughter church building. The Mill Creek congregation 
merged with that daughter congregation. Later, a new congregation 
started a different Mill Creek Baptist Church, not descended from the 
original church by that name. The original location of that second 
church is mapped in OSM as "Mill Creek Church", ID 356845407. This 
second congregation later moved to a different location, which is not 
currently mapped in OSM.  I would like to map it as well. So, I have 
questions:

How do I tag two different historical churches with the same name, but 
not the same identity, that are located a few miles apart? Would adding 
"established <x>" and "established <y>" as part of the names of the two 
different historical locations be an appropriate technique, with the 
third node for the current church's location also having the 
"established <y>" as part of the name? I would like to have some way to 
indicating the linkage.
