But healthcare=pharmacy was never proposed.

It was added by a preset in iD.

At the time the key “healthcare=“ was proposed, the idea of changing
amenity=pharmacy was opposed by several mappers during the RFC period (in
2010), so the idea to change to shop=pharmacy was dropped.
