There IS such a thing as a "compounding pharmacy," at least in the USA, I don't know about other countries. This is a licensed pharmacy, with licensed pharmacists, who create bespoke / custom medicines from the raw materials of other licensed drugs (what the USA calls "Scheduled" drugs), based on a medical doctor's "custom crafted" prescription. Yes, they are rare, but they do exist, as the practice of medicine agrees there is a medical need for them. They are legally recognized as not only a "pharmacy," (and all that entails), but as a "compounding pharmacy," at least in my state.
Kevin's suggestion of 'pharmacy:compounding=yes/no' seems like excellent tagging here (in the USA). If the concept exists in England and other countries (I'm nearly certain it does, simply because of the medical need), it would be good to hear / read what they are called. However, I think that dart on the dartboard of Kevin's hits (or comes very close) to a bullseye.
