if you want to add so much information, I think it would make sense to
create an amenity=toilets object instead of sticking it on the POI, this
will give information on its position and will also allow to deal with
the case of 2 toilets not having the same characteristics

> toilets:wheelchair_door_width                (for the door width of the
> toilet)

the correct namespace separator is ":", no the "_"
so if you want to glue all info on the poi
toilets:wheelchair:door:width
or an amenity=toilets + wheelchair=yes + door:width=*

> toilets:wheelchair_space_side                   (for the space next to
> the toilet)

toilets:wheelchair:space_side

> toilets:wheelchair_space_front                 (for the space in front
> of the toilet)

toilets:wheelchair:space_front

> shower:wheelchair                                       (for a
> wheelchair accessible shower, answer with yes/no)

or it's own object with amenity=shower + wheelchair=*

that's fine

this will severely limit you unless you introduce some very twisted rules.
a poi for example has 2 amenity=toilets, which do not have the same
characteristics in terms of space, door width, etc
How are you going to fill this in on a single object?
