I use the java app mkgmap to generate them (after a little osmosis
processing), and all I've done is modify the mkgmap configuration files
to reset the levels of all road types to about 20 ro 21 rather than 24.
It's a little rough, and basically useless in metro areas, but works a
treat out in the rural areas. I might have a go at refining it a little
bit in the future.

I got really lazy, and the whole process is now automated, so the maps
are generating every morning at 3am, except tue and thurs mornings
(don't need to completely destroy my bandwidth each month).
