I notice that the new stable release of JOSM (version 3094) seems to 
have settled the question of how to tag shared paths by fiat.

It offers a way type of "Combined Foot- and Cycleway", which sets this 
tag combination:

bicycle=designated
foot=designated
highway=path
segregated=no

The default rendering on OSM is very similar to "highway=cycleway", 
except that it's a purplish shade of blue.
