I usually look at a few examples that already exist such as:
artist=William Haywood
historic=memorial
material=concrete
name=Scout War Memorial
source=survey
start_date=1924_07_27
wikimedia_commons=Category:Scout Memorial Birmingham
wikipedia:architect=en:William Haywood(architect)
listed_status=

or

memorial=blue_plaque
