If you use the term "district", then indeed, Antwerp is the only 
municipality in Belgium that has districts.

District is just a special form of "deelgemeente" that has its own 
district counsel which means extra elections. So the term we'd need to 
use for OSM would be deelgemeente.

But yeah, not all municipalities have deelgemeentes. I thought Stabroek 
has two though: Hoevenen and Stabroek itself. Municipalities like 
Borsbeek, Wijnegem or Wommelgem have no deelgemeentes.

Yeah, if multiple values were supported, which they aren't at the
