I don't think you have to select layers because they are specified in
the URL.  In other words, the program seems to automatically check for
you.  For instance, the URL includes "designated_areas" and that layer
is checked on my system.  However, you can check whichever ones you want.

In my case, they are:

Projection: EPSG:4326
Tile it: unchecked       (I don't know what this is for)
Zoom levels: 0           (I don't know what this is for)
Image format: image/png  (same value as in the URL)
Styles: blank            (I don't know any appropriate values for this)
