It is possible to merge titles at the land registry, at their
discretion, but I suspect many freeholders don't bother doing this
until they need to sell the property.
There are a few parks in London which still show long-demolished
parcel boundaries of old bombed-out housing from the war. I assume the
council hasn't bothered merging them as they're not likely to be sold.
You do also see boundaries in the middle of other large parcels
sometimes.
