For simple error reporting we have OpenStreetBugs. There are two
instances at http://openstreetbugs.schokokeks.org and
http://openstreetbugs.appspot.com. AFAIK they are synchronized, so any
bug entered on one will show up on the other. For haiti there is also
http://ose.petschge.de/ which shares most of the codebase with
OpenStreetBugs but is meant for the collection of "in the field
reports". As such it does NOT synchronize with the regular OpenStreetBug
instances.
