For Amsterdam and similar cities that have a segregated cycleway  
system, I think the following is a nice simplification of the above:

<----- highway=cycleway; cycleway=segregated; foot=yes
<----- road
<-----> tram (only oneway when it separates)
------> road
------> highway=cycleway; cycleway=segregated; foot=yes
