Well, I finally studied up enough on Overpass's messed-up query language
to construct a reasonable script to ferret out junctions with no REF tags.
This mostly finds trail intersections in parks, but there are a few points
tagged as junctions outside of those too.  I have a first cut at a query
permalinked as   https://overpass-turbo.eu/s/1CxT   -- such links seem to
get permanently created over there whenever you do "share".  I see how
Overpass-turbo can be a seriously powerful and useful tool.

My efforts in the Fells are nearing completion; I'm having to fix a few items
that others did [recently, even] and didn't add "ref".  Looks like Glen et al
missed a handful in Harold Parker.  The Blue Hills doesn't seem to have any
"ref" tags at all, along with many other parks with labeled intersections.  If
a trail connection is supposed to have an intersection number but isn't in
OSM as a junction yet at all, this query won't find it, but visual inspection
after zooming in and comparison with official park maps could maybe sniff some
of that out.
