So it sounds like the process is a combo of contacting the mapper and 
sending an send an email to the DWG? Do they handle these cases gently 
with new users so as not to discourage future mapping?

In a case like this, a reversion probably needs to be done. In the past, 
I have contacted the mapper and also asked this list for help from 
someone experienced with reversions to do the reversion and someone on 
the list checked it out and did the reversion.

I suppose overall, I am asking is there a formal / accepted process for 
handling obvious mistakes and potential reversions?

I think this could be a job for the Welcoming Committee. I'm not sure if 
there is one - I think I remember reading something about that topic on 
one of the mailing lists. New users sometimes don't know all of "the 
rules", or need to be reminded, and should be guided along when they 
make mistakes. I suppose that's part of the overall community ethos as 
well, i.e. reach out to people and communicate with them as we find issues.
