[Talk-us] exit_to vs destination

CC'ing in [Talk-ca] since they (at least in Ontario) don't even use the exit_to or destination tags and put everything in the 'name' tag on 'motorway_junction's.  Thinking it would be a good time to see if we can get them on the same page as well.
