[OSM-talk] OSM Survey - Software to use?

Hello,

I'm trying to put together a use survey for OSM, in relation to my MSc
project.

Can anyone recomend some simple, open source, online survey package that I
can host on my site?  I've been looking at SourceFOrge's offerings like Web
Survey Toolbox, but they seem overly complex for what I want (about 30
questions and minimal analysis).  If anyone has used online surveys before,
I'd appreciate some tips.
