My understanding is that the key isn't called 'access',  but you have
individual 'foot', 'horse', 'bicycle' and 'motorcar' keys to represent
different types of access, all grouped under the 'access' category. 
e.g you could have foot=yes, horse=permissive if it was a public footpath 
which the landowner had given discretionary access to (there are several real 
examples of this)

But I think the second edition of Map Features is going to sort out these
sorts of ambiguities, along with dealing with way widths.
