Thought I'd have a go. Based strictly on Richard's sketch.

It's just a first draft for comments.

Works best in Firefox at the moment. Doesn't look too bad in IE but I'll
use background images to do the rounded corners, I think.

The images are a bit small. I could do with some slightly larger ones 
but I got these from the other draft (http://dyoh.com/osm-draft-index.html).
