Hm. I'm actually building that at present from my own data, that of
other like-minded people, and US public domain
(http://www.systemeD.net/carto/ukbasemap.html) - but as PD rather than
CC-SA. OSM's licence is print-unfriendly so I wouldn't recommend
anyone spends too much time on doing the same from OSM data. :(
Ordnance Survey does actually have an liberal licence policy for
diary-type maps anyway:
"1:1 000 000 scale mapping For publications that contain 'redrawn
mapping' at a scale of 1:1 000 000 or less, a licence fee may not be
payable provided the mapping is sourced from 1:1 000 000 scale
Ordnance Survey mapping with a
similar specification to Ordnance Surveys MiniScale dataset."
