This seems more a problem of a cache somewhere in between David and the
dev server that is not refreshing the tile.

Might be the browser cache, a web proxy at his location or at his ISP.

I vote for a header for the pngs that invalidates the cache after 6
hours, since that is longer than the longest time I'd imagine someone
(single one person) looking at the same tile(s) over and over.

Since the proxy are not at the OSM end of things a long caching time
will not help reducing the load, but instead lead to stale data being
"served".
