Can I confirm that is it necessary for both:

1) The main area to have role 'outer', and the hole to have the role 
'inner'; AND
2) The main area to be anticlockwise, and the hole to be drawn clockwise.

Not only does it seem strange to have to do two things (I could accept doing 
either 1, or 2), but as far as current rendering on the tiles at home layer (2) 
does not actually seem necessary (I'm awaiting re-rendering of the Mapnik 
layer over the next day to see how that looks).
