Assume you're DrMark from the wiki.

So are people more in favour of using amenity=clinic to mean *just* 
what's described in http://en.wikipedia.org/wiki/Polyclinic and nothing 
smaller? Specifically not a 1-man doctor's surgery or a 1-specialty 
dentists' practice.

If there's strong support for this, I've no objection to making lots of 
little amenity=dentist-style tags for the smaller practices and forgoing 
the merge.
