As for the non-Latin characters that appear, this is probably because you have
multiple languages specified in your browser language preferences, and the
current version of Nominatim (with the currently tagged data in OSM) is unable to
work out that the English name should be used.

The hierarchy of place names is a different subject which I'll leave others to
answer, suffice to say that the USA is not the only country that has problems.
